Application and instrument of approval for change of trade description to be used by a person in a position of management or control of the export registered establishment where the alteration to trade description is to be made, to apply to the department to alter the trade description of products.

This form is applicable to the following commodities:

• Red meat and meat products
• Game meat and meat products
• Poultry meat and meat products
